I'm sure that if I ask you what comes to mind with the concept of 'sexual health,' the last thing you'd think of is... tight underwear? And it's that often, everyday factors that can have a direct impact on sexual and reproductive well-being are overlooked.

One of those elements is the type of underwear used. Although it may seem like we shouldn't worry about it, the use of tight clothing, especially very tight underwear, has generated quite a few concerns and questions: can it affect male fertility? Does it influence erections or sexual desire? As I know these can be important questions for you, in this article, I am going to share the available scientific evidence and discuss some common myths that persist on the subject.

Why is it considered that tight clothing could affect fertility?

The main argument is based on the link between testicular temperature and sperm production . Let me explain: the testicles are located outside the body precisely because they need a slightly lower temperature than body temperature to function correctly. If those degrees rise, it can affect spermatogenesis (that is, sperm production).

This is where tight clothing comes into play, as by keeping the testicles closer to the body, it could increase their temperature . This has led many experts to suggest that tight underwear could alter semen quality by creating a warmer environment than necessary. You might see it as minimal, but keep it in mind.

Scientific Evidence: Myth or Reality?

Several studies have attempted to answer this question. One of the most cited is the one published in Human Reproduction (Minguez-Alarcon et al., 2018), which analyzed more than 650 men and found that those who wore tight underwear had a slightly lower sperm concentration compared to those who wore loose underwear. However, it was also noted that hormone levels and other seminal parameters remained within normal values.

This suggests that, although there is an association between tight clothing and semen quality, the impact is not as dramatic as some myths suggest . In general, there is no conclusive evidence that habitually wearing tight underwear causes infertility, but it could be a factor to consider in men with difficulties conceiving. So, don't panic!

Beyond Fertility: Other Effects on Sexual Health

The concern about tight clothing is not limited to fertility alone . Although it might sound like a matter of preference, it's true that many men report feeling discomfort, increased sweating, or irritation in the genital area when wearing overly tight garments. And in part, it makes sense, as constant friction can lead to dermatological problems, such as dermatitis or fungal infections, which indirectly affect sexual life.

Furthermore, excessive heat in the genital area can not only affect fertility but also temporarily influence erectile function. Although there is no direct evidence linking the use of tight underwear to erectile dysfunction, there is a correlation between elevated testicular temperature and lower testosterone production , a fundamental hormone for sexual desire and erection (Setchell, 1998).

Lifestyle, Temperature, and Fertility

Okay, up until now you might be thinking it's not that big of a deal. But it's important that we understand that the type of underwear doesn't act in isolation. There are other factors that influence testicular temperature and, therefore, reproductive health:

  • Frequent use of laptops on the lap .

  • Sitting for many hours (due to work or long journeys).

  • Exercising in very tight synthetic clothing without changing quickly .

  • Frequent saunas, jacuzzis, or hot showers.

All these factors, combined with the habitual use of tight clothing, can have a cumulative effect on semen quality. If you keep adding grain by grain, in the end it does have an effect on your sexual health.

Tips to Preserve Sexual and Reproductive Health

  1. Alternate types of underwear : If you're someone who feels most comfortable in briefs, you can alternately use loose-fitting clothing, such as cotton boxers, as a healthy option, especially in warm climates or while sleeping.

  2. Ventilate the genital area : No, I'm not asking you to go without underwear all the time, but something as simple as sleeping without underwear or wearing loose pajamas helps maintain a suitable temperature.

  3. Avoid overheating : Do not use laptops directly on your lap and take breaks if you are sitting for many hours.

  4. Good hygiene and natural fabrics : Wearing breathable garments helps prevent irritation and infections.

  5. Consult if you have difficulties : If there are suspicions of fertility problems, the ideal thing to do is to see an andrologist for a complete evaluation.

Conclusion

The connection between wearing tight clothing and male sexual health is more complex than it seems. While very tight underwear is not solely responsible for infertility or sexual dysfunction, it can be a factor to consider, especially when combined with other lifestyle elements that raise testicular temperature.

The key lies in balance: it's not about completely abandoning certain types of underwear, but about being aware of the cumulative effects and deciding which things or changes favor your comfort, health, and sexual well-being . Good sexual health begins with small, everyday details that we often overlook.
